Homeopathy, pseudoscience and the anti-vaccine movement: why we work with scientific evidence
In clinic we often hear legitimate questions about homeopathy, vaccines or "miracle" treatments. The short answer is that the available evidence is clear. The long answer requires distinguishing between what a well-designed study can show and what a claim dressed up as science can appear to show.
Why this conversation matters
Pseudoscience rarely walks into the clinic under its own name. It arrives disguised as "footwear that corrects your gait", "a miracle technique that realigns your energy", "supplements that regenerate cartilage" or "one session that undoes years of pain". The common thread is always the same: a strong promise with no quality placebo-controlled trials behind it. Working with primary evidence is not an academic whim; it protects patients from treatments that are ineffective, costly and sometimes harmful.
Homeopathy: when trials are properly controlled, it does not outperform placebo
The study that compared homeopathy with conventional medicine
Study published in The Lancet that matched 110 placebo-controlled trials of homeopathy with 110 of conventional medicine by condition and outcome. Restricting the analysis to larger, higher-quality trials: homeopathy OR 0.88 (95% CI 0.65-1.19), no effect distinguishable from placebo. Conventional medicine: OR 0.58 (95% CI 0.39-0.85), a significant effect.
The authors' literal conclusion: there is weak evidence for a specific effect of homeopathy and strong evidence for a specific effect of conventional medicine. The finding is compatible with the idea that the clinical effects of homeopathy are placebo effects. Add to this the basic physical chemistry: the usual homeopathic dilutions (30CH, 200CH) mean that the probability of a single molecule of the active principle remaining in the bottle is practically nil.

The Wakefield case: when fraud becomes an epidemic
Twelve years of anti-vaccine fraud, step by step
A walkthrough of the case published in Presse Médicale: in 1998 Wakefield publishes in The Lancet on 12 children with autism and the MMR vaccine; in 2010 the UK General Medical Council finds that the clinical records did not contain the evidence described and The Lancet retracts the paper; in 2011 the journalist Brian Deer reveals in The BMJ that Wakefield had been funded by an anti-vaccine lobby, an undeclared conflict of interest.
Wakefield was struck off the UK medical register. Yet the damage persists: measles outbreaks return in regions where MMR coverage falls below the herd immunity threshold. Vaccination works; so does the pseudoscience attached to it, at spreading disease.

Why parents still hesitate over MMR, two decades on
Systematic review of 115 articles on MMR vaccine refusal. Fear of autism is the most cited reason among hesitant parents, two decades after Wakefield's retracted paper. Refusal clusters in middle-to-high income areas, among university-educated mothers who favour narratives from the internet and social media over their paediatrician's information.
The damage done by scientific fraud does not end when the paper is retracted; it keeps circulating as a cultural meme. The clinical consequence: countering misinformation requires intersectoral, multifaceted interventions, not just more data. The honest one-to-one conversation in clinic remains, to this day, one of the most effective tools.
