What we measure in a biomechanical gait assessment

A poorly aligned foot can cause pain much further up: knee, hip or lower back. That is why the assessment does not stop at the foot, it looks at the whole movement chain.

Computerised gait analysis

Pressure platforms and high-speed video capture, to the millimetre, how you distribute your weight when you walk and when you run. We pick up imbalances the human eye cannot see.

3D kinematics of the lower limb

Three-dimensional movement analysis assesses the full kinematic chain: ankle, knee, hip and lumbar spine. That is how we find where the problem really starts, not just where it hurts.

Photogrammetry

Postural photogrammetry: measuring to the millimetre

Podiatric photogrammetry measures alignment and differences in length between the legs (leg length discrepancy) from calibrated images, with a reliability equivalent to that of the reference methods in biomechanics. It is quick, non-invasive and involves no radiation.

At Avanza Salud we use it above all to assess leg length discrepancy and to decide, case by case and against clinical thresholds, when to compensate with an orthosis and a measured heel raise. An undiagnosed leg length discrepancy can lie behind low back pain, hip overload or repeat injuries in runners.

History and examination

We talk about your pain, your activity and your goals. Joint and muscle examination on the couch, both non-weight-bearing and weight-bearing.

Movement analysis

You walk and, where appropriate, run on the Podonexus walkway: pressure platforms and high-speed video record every footfall.

Measurements and photogrammetry

We measure alignment and any leg length discrepancy with calibrated postural photogrammetry, to the millimetre and with no radiation.

Diagnosis and plan

We explain the findings and design the treatment: exercise, footwear, physiotherapy or custom Podonexus orthoses if they are indicated.

Podonexus orthoses are designed from the data in your assessment and made entirely in the Canary Islands, matched to your anatomy, your activity and your goals. Common questions about the biomechanical gait assessment

What does a biomechanical gait assessment involve?
It is a complete assessment of how you walk, run and move. It combines the clinical examination on the couch with computerised gait analysis on the Podonexus walkway, pressure platforms, high-speed video and postural photogrammetry. With that data we design the treatment: an exercise programme, footwear modifications or custom Podonexus orthoses if they are indicated.
Do I need a biomechanical gait assessment if I have no pain?
Not always. It is particularly worthwhile if you run or do impact sport regularly, if you notice unusual wear on your shoes, or if you have a history of repeated injuries. In children it lets us pick up and correct developmental changes before they become established.
Do you carry out biomechanical gait assessments on children?
Yes. We see children and adolescents from the age of 3. The assessment is adapted to the growing foot and, when support is needed, we use the Nexus Kids range of paediatric orthoses.

Is it true that orthoses are hard and take some getting used to?
That reputation has a real basis, but it applies to orthoses made from rigid materials such as polypropylene or high-hardness resins, which we do not work with. Podonexus orthoses are made from materials that mould: the orthosis adapts to your foot, not your foot to the orthosis. In practice, people who try them for the first time usually do not want to take them off, they are that comfortable.
Do I need to change my footwear to wear orthoses?
The orthosis takes up physical space inside the shoe. With very tight footwear you usually need half a size or a full size larger so that the foot and the orthosis fit comfortably. It is worth bearing that in mind when choosing the shoes you will wear them with; when we fit them we advise you on which shoes suit them best.
